This is the 2023 RSTC Approved Medical form for Scuba Diving Activities. It must be filled out before any dive training is begun. A Physician's Approval is required for any diver who has a potential or actual medical condition (if any question listed on Page One has an affirmative answer, AND the Page Two detail then requires a physician's release) that is deemed adverse to SCUBA diving.
